Always ensure you obtain full consent from your client before providing any cosmetic treatment. You must inform the client of all the risks and benefits as well as any potential side effects. Clinicians must ensure the client has read and understood this information before they consent to the treatment. Providing this information both verbally and in hard copy will assist with this. Allow the client time to absorb the information and to ask any questions.
Each informed consent must be documented and signed. Clinical forms such as patient consent forms should be reviewed and updated if required to ensure all documents contain the current information or warnings.
These documents must be filed in the patient treatment history and must be kept for up to 10 years.

A list of all the files available for download will be displayed.
Click the document you require and they will download to your computer.
Edit and update the documents and forms for use in your clinic.
