Always ensure you obtain full consent from your client before providing any cosmetic treatment. You must inform the client of all the risks and benefits as well as any potential side effects. Clinicians must ensure the client has read and understood this information before they consent to the treatment. Providing this information both verbally and in hard copy will assist with this. Allow the client time to absorb the information and to ask any questions.

Each informed consent must be documented and signed. Clinical forms such as patient consent forms should be reviewed and updated if required to ensure all documents contain the current information or warnings.

These documents must be filed in the patient treatment history and must be kept for up to 10 years.

Clinical Patient Documents and Processes for Botox Cosmetic Treatments

Each patient will require a consultation with the clinician to ensure they understand the botox treatment and they have realistic expectations of what botox can achieve. During the consultation all the complications that are associated with the treatment must be explained to the patient.

The BOTOX Patient Medical History

Before each patient is treated with BOTOX.

The clinician must ask the patient to complete and sign a medical history questionnaire. This will usually be a tick sheet with requests for further information about any questions that require more information to ensure a correct assessment of the patients suitability for the Botox treatment.

The medical history contains questions about cautions and contraindications for the specific treatment which include, neuro musculature disorders plus lifestyle, pregnancy, plus any medications the patient may be using or previous cosmetic treatments, surgery and adverse reactions. It is essential that the clinician administering the botox treatment and the prescriber (if they are not providing the treatment to patient) reviews the patient and medical history thoroughly. Do not forget the signed informed consent which is also required and to give your patients the Botox post treatment instructions verbally and in writing.

Medical history, informed patient consent and patient treatment records for BOTOX are a medical and legal requirement, these documents must be retained. Please review the Department of Health web site for more information about how long you need to keep patient records, you should also review your insurance companies recommendations and don’t forget to take the patients before and after photos (2 weeks post treatment review for after photos).
